Per Diem Hospice Social Worker | Flexible Scheduling

We are seeking a per diem (as needed) Licensed Social Worker for the Chester County, PA area. Offering competitive pay and flexible scheduling. Responsibilities This is a professional position responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families, including completing the comprehensive psychosocial assessment and regularly assessing psychosocial needs. The social worker also completes the bereavement risk assessment of the family and provides counseling relative to anticipatory grief issues. Upon completion of the assessment, a psychosocial plan of care is developed with the Interdisciplinary Team (IDT) and the patient/family, and is continually revised and reassessed for psychosocial and potential bereavement issues. Supportive counseling Problem solving and referrals to community resources Pre-bereavement counseling and support Education, Training, and Certifications Master’s Degree in Social Work (MSW) from a graduate school of social work. Current license as a Social Worker by the Pennsylvania Board of Social Work Examiners. Licensed driver with an automobile in good working condition and automobile insurance in accordance with PA laws. Skills and Knowledge Visual/hearing ability sufficient to comprehend written/verbal communication. Excellent verbal communication skills. Ability to effectively manage stress. Manual dexterity sufficient for writing, keyboarding, and telephone operation. Ability to work in a variety of settings and conditions in patient’s homes and facilities. Experience Understanding of Hospice philosophy, principles of death/dying. Hospice or home care experience desired. Ability to assess patient needs and formulate individualized patient care plans to meet those needs. Ability to work in a field setting and exhibit sound judgment.
