Key Responsibilities

• Provide physical therapy services to patients in a home health setting within an assigned county territory
• Conduct evaluations, develop individualized treatment plans, and deliver skilled therapy interventions in patients' homes
• Travel throughout the assigned territory to complete patient visits, averaging approximately 200–400 miles per week
• Document patient care accurately and in a timely manner using the designated electronic medical records system
• Participate in an initial in-office orientation period prior to beginning field assignments

Required Qualifications

• Active Physical Therapist (PT) license in the state of Arizona
• Ability to travel throughout Pinal County as required by caseload
• Valid driver's license and reliable personal transportation
• Proficiency with or ability to learn electronic medical records platforms

Preferred Experience / Skills

• Prior home health physical therapy experience strongly preferred
• Familiarity with HCHB (HomeCare HomeBase) EMR system
• Strong time management and self-directed work skills suited for an independent field-based role
