Requirements: Most recruited for:
• Case Management experience
• Midas/InterQual use
• Licensed Masters

Generate 4-5 concise, professional bullet points relevant to the specific profession and setting.

• Manage patient care coordination and discharge planning for diverse hospital units.
• Conduct face-to-face interviews with patients and families on a daily basis.
• Utilize MediTech EMR system to document and review patient information.
• Collaborate with healthcare team members to facilitate patient progress and resource allocation.
• Follow hospital policies and guidelines, including weekend rotations and scrubs attire.
