Responsibilities Conduct comprehensive assessments to develop individualized care plans for hospice patients. Administer medications and treatments as prescribed, ensuring patient comfort and safety. Provide emotional support and education to patients and their families regarding end-of-life care. Collaborate with interdisciplinary teams to coordinate patient care and services. Document patient progress and care interventions accurately in electronic health records. Participate in on-call rotations to provide after-hours support as needed. Monitor and manage symptoms to enhance the quality of life for patients. Advocate for patients’ needs and preferences in all aspects of care. Requirements Valid RN license in the state of Texas with hospice experience preferred. Strong clinical assessment skills and knowledge of pain management protocols. Excellent communication and interpersonal skills to engage with patients and families. Ability to work independently and as part of a collaborative team. Compassionate demeanor with a commitment to providing high-quality patient care. Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) preferred. Flexibility to work various shifts, including evenings and weekends. Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
