Position Name: Registered Nurse HomeHealth Location: Sturgeon Bay, Wisconsin Specialty: HomeHealth Start Date : ASAP EmploymentType: Full-time, Travel,Contract, Temporary Pay Rate:Range $2000 to $2080 Weekly Gross Pay based on a 40-hour work week (Includes Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers) MileageReimbursement: $0.49/mile for distancecovered between patient home appointments Unit: Home Health Position Requirements: Registered Nurse: 2 years - Required Recent/Current Home Health Nursingexperience: 1 year within the past 3 years - Required OASIS experience - Required Home Care Home Base (HCHB)charting experience - Required Must have valid Driver’s Licenseand Personal Auto Insurance Education, Licenseand Certification Requirements: ASN or an equivalent – Required BSN or an equivalent – Highlypreferred RN License for State applying or Compact if accepted in the state - Required BLS Certification from AHA orAmerican Red Cross - Required Job Description: Provide and coordinate care forassigned patients in comfort of their homes. Develop and implement nursingcare plans including Start of Care and admissions as assigned. Support patient recovery andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy Empower patients and families toreach healthcare goals by educating them on disease management Case management and provideclinical oversight to interdisciplinary Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures. Ensure physicians’ orders arelegible if taken verbally and transcribed appropriately per policy andprocedure. Ensure patients’ status isassessed on an ongoing basis and pertinent information is gathered anddocumented. Notify physicians of significantchanges in status Document and complete chartingfor all patient visits within 24 hours.
