North Alabama Medical Center is a 263 bed facility offering the community's only open heart surgery site, a complete line of women's and children's services and a complete line of medical and imaging services. We are a Certified Chest Pain center and recognized as a Primary Stroke Center of Excellence by the American Heart/American Stroke Association. North Alabama Medical Center's 24-hour emergency department serves as the primary referral center for major trauma cases in Northwest Alabama. Where We Are:

Education: Bachelor's Degree preferred or 3 years of experience;

Required Skills:
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Essential Functions:
Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care
Keeps track of all Medicare forms and consent forms
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions
