A growing home health organization is seeking a Registered Nurse to provide skilled nursing services to patients in their homes. This role offers the opportunity to work one-on-one with patients, develop meaningful clinical relationships, and deliver personalized care in a home-based setting.
Schedule:
- Full-time, 40 hours per week
- Day shift: 8:30 AM - 5:00 PM
- Every fourth weekend
- Two holidays out of six annually
- No on-call responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Deliver skilled nursing services in accordance with physician orders, clinical standards, and regulatory requirements.
- Monitor patient progress and adjust care plans as needed based on changes in condition.
- Perform medication reconciliation and collaborate with providers regarding treatment plans.
- Ensure timely and accurate clinical documentation within established organizational guidelines.
- Provide patient and caregiver education to support treatment goals and promote positive outcomes.
- Coordinate care with physicians, therapists, home health aides, and other interdisciplinary team members.
- Manage visit schedules and maintain compliance with payer authorization requirements.
- Communicate patient status updates and care plan changes to appropriate healthcare team members.
- Supervise home health aide staff and complete required supervisory visits.
- Maintain current knowledge of nursing standards, best practices, and regulatory requirements.
- Meet productivity expectations and complete assignments independently and accurately.
- Perform additional duties as assigned.
- Associate Degree in Nursing or Nursing Diploma.
- Minimum of six (6) months of Registered Nurse experience OR at least one (1) year of Licensed Practical Nurse experience.
- Experience using computers and electronic documentation systems.
- Active New Jersey Registered Nurse (RN) license.
- American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers certification.
- Bachelor of Science in Nursing (BSN).
- Home health or hospice nursing experience.
- IV Certification.
