This Monday through Friday, day-shift position (8:00 a.m. to 4:30 p.m.) focuses on care coordination, symptom and medication management, treatment oversight, diagnostic and specialty care navigation, and real-time support of provider and patient care needs. Success in this role requires strong communication skills, exceptional attention to detail, and the ability to manage multiple pathways of care across the healthcare continuum. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.Requires that the RN has graduated from a nationally accredited nursing program. Exceptions: Candidates who have not graduated from a nationally accredited program may be considered if they possess more than two years of relevant clinical experience.
