Your Daily Responsibilities
- Conducting thorough assessments of patients, ordering essential diagnostic tests, and tailoring individualized care strategies
- Diagnosing and managing acute, chronic, or episodic health conditions independently or in collaboration with physicians
- Educating patients and their families on wellness, prevention, and proactive disease management
- Prescribing necessary medications and therapies in accordance with your practice scope
- Engaging in ongoing patient follow-up and chronic care coordination
- Working collaboratively with interdisciplinary teams to ensure cohesive patient care
- Maintaining accurate and thorough documentation using electronic medical record (EMR) systems
- A Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Graduation from an accredited Nurse Practitioner program
- Active Nurse Practitioner licensure in the state of California
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, or ability to obtain within 90 days
- A strong clinical judgment combined with a warm bedside manner
- Exceptional communication abilities with patients, families, and team members
- Flexibility in fast-paced, patient-centered healthcare settings
- Proficiency with EMR systems and comfort using telehealth technology
- A collaborative attitude and a commitment to evidence-based practices
