Exciting perks and competitive pay await you!/ph3Qualifications:/h3ulliGraduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing./liliMinimum of one (1) year experience in nursing, preferred./liliAcceptance of the philosophy and goals of this Agency./liliAbility to exercise initiative and independent judgment./li/ulh3Principal Responsibilities:/h3ulliUnderstands and adheres to established policies and procedures./liliImplements the nursing care plan for each patient./liliProvides nursing services, treatments, and diagnostic and preventive procedures as assigned./liliInitiates preventive and rehabilitative nursing procedures as appropriate./liliObserve signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patients physical or emotional condition./liliTeaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home./liliEvaluates with registered nurses the effectiveness of the LPNs/LVNs nursing service to the patient and family under the guidance of the registered nurse./liliMaintains accurate and complete records of observations, treatments, and care of patients./liliParticipates in medical record audit as assigned./liliAttends staff meetings, patient care conferences, and in-services as scheduled./liliTakes on-call duty, nights, weekends, and holidays as assigned./liliIs responsible for: ulliSubmitting any changes in schedule to the Director of Clinical Services/Clinical Manager daily./liliParticipating in patient care conferences to discuss the need for the involvement of other members of the health team, such as physical therapists or speech-language pathologists/li/ul/liliPrepares clinical and progress notes./liliAssists the physician and RN in performing specialized procedures./liliPrepares equipment and materials for treatments./liliObserve the aseptic technique as required./liliAssists the patient in learning appropriate self-care techniques./li/ulh3Preferred Qualifications/h3ulliHome care and home health background./liliKnowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements./liliKnowledge of the local healthcare market and community outreach strategies./liliKnowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR)./liliClean drivers license with a clean driving record (Preferred)./li/ulh3Benefits/h3ulliHealth, dental, and vision insurance./liliPaid time off and holiday pay./liliRetirement plan./liliMileage reimbursement./liliFlexible schedule./liliLife insurance./li/ulh3Schedule/h3ulliMonday to Friday/liliWeekends as needed/li/ulh3Work Location:/h3pHybrid/ppGranite Peak Home Health provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
