Requirements

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• CHPN certification - Preferred
• Proof of current CPR certification - Required
• Current driver's license - Required
• Reliable transportation and insurance - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelors of Science in Nursing - Preferred

Responsibilities

• Assess and respond to the needs of home health and/or hospice patients in various situations.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure continuity of care.
• Educate patients and families on care, medications, and safety practices.
• Supervise and provide clinical direction to assistive personnel.
• Monitor assigned cases for compliance with payer requirements.
• Provide communication as a liaison to patients, families, and interdisciplinary team members.
