Care Manager

Job Title
Location: 75% Travel to members' homes, ideally located in Central KS - Salina, Hutchinson, Great Bend, Pratt, and surrounding areas
Duration: 6 months, possible extension
Shift: 8am-5pm, with possibility of some evening hours due to business need
Responsibilities
- Support members in foster care and coordinate services to ensure their needs are met
- Conduct 48-hour in-person visits following a placement change for the member
- Participate in 7-day team meetings following a placement change for the member
- Conduct telephonic outreaches based on member's acuity
- Perform in-person visits based on member's acuity
- Complete annual assessments
- Attend in-person visits that may take place after 5pm
- Participate in weekly team meetings
- Track completion and timeliness of Health Risk Assessment and Plan of Service
Requirements
- Master's degree in Behavioral Health, Social Work, or a degree from an Accredited School of Nursing
- Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience
- Minimum of 1 year of experience coordinating services across child welfare and related systems of care
- Strong communication, high-quality documentation, adaptability/flexibility, and solution-focused approach
