Job Summary:

About Nyack Ridge Rehabilitation & Nursing Center:

Our highly regarded therapy department is situated on our first floor, equipped with a state-of-the-art rehabilitation gym and a peaceful subacute wing for short-term patients transitioning from hospital to home.
Responsibilities:
Provide direct patient care, including vital signs monitoring, wound care, and medication administration
Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Document patient progress and maintain accurate records
Maintain a clean and safe environment for patients and staff
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Must possess a Certificate in Nursing (LPN) with 2+ years of experience
