LPN - Population Health Specialist
Join a dynamic healthcare team as a Licensed Practical Nurse specializing in Population Health initiatives. In this role, you will support system-wide efforts to enhance patient wellness and care coordination across a regional healthcare network in the Southern United States.
Key Responsibilities:
- Assist with the implementation and management of population health programs aimed at improving patient outcomes.
- Oversee scheduling processes, prioritizing wellness visits and closing care gaps to ensure timely patient care.
- Collaborate with managed care teams to analyze and distribute reports that support value-based care arrangements.
- Utilize strong EMR skills and Excel proficiency to streamline workflows and data management.
Education & Experience:
- Licensed Practical Nurse (LPN) credential required.
- Experience in primary care settings with a solid understanding of clinical procedures.
- Demonstrated ability to manage detailed tasks with precision and excellent organizational skills.
Requirements:
- Exceptional communication skills to effectively liaise with healthcare providers and team members.
- Detail-oriented mindset to ensure accuracy in scheduling and reporting.
