Working remotely full-time, the New York Licensed Nurse Case Manager will conduct telephonic and in-person assessments, develop individualized care plans, and coordinate services for members enrolled in Managed Long-Term Services and Supports (MLTSS). Key responsibilities Conduct comprehensive telephonic assessments and in-person evaluations to determine medical needs Develop and authorize individualized care plans within MLTSS/FIDE program benefits Document all assessments and interactions accurately in the electronic health record Required qualifications Active, unrestricted RN license in the state of New York Minimum of an associate degree in Nursing Case Management Certification (CCM) preferred
