Case Manager: Empower Recovery & Community Access
Basecampmed
Basecamp Recovery Center is seeking a dedicated Case Manager in Whitehall/Columbus to link patients with housing, employment, and community services. You will advocate for access to needed supports and coordinate assessments, childcare, transportation, finances, and education within a supportive team. You will maintain detailed progress notes, support treatment goals, and collaborate with clinicians while engaging with diverse clients. #J-18808-Ljbffr Basecampmed
