• Provide physical therapy assessments and develop treatment plans for patients in a long term care setting.
• Implement therapeutic interventions to improve patient mobility and function.
• Monitor patient progress and adjust treatment plans accordingly.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Document patient progress and maintain accurate records in compliance with facility policies.
