About the Role:

The Physician Assistant or Nurse Practitioner at Benemed Physician PC/ Zambito Heart Center will play a crucial role in delivering high-quality healthcare services to our patients. This position involves conducting comprehensive assessments, diagnosing conditions, and developing treatment plans tailored to individual patient needs. The successful candidate will collaborate with physicians and other healthcare professionals to ensure optimal patient outcomes and continuity of care. Additionally, the role includes educating patients about their health conditions and promoting medication adherence to enhance their overall well-being. Ultimately, this position aims to improve patient health through effective management of cardiovascular diseases and other medical conditions in a supportive and patient-centered environment. A must have bilingual proficiency in English and Spanish.

Minimum Qualifications:

• Current Nurse Practitioner or Physician Assistant license in New York State.
• Master's degree in Nursing or Physician Assistant studies.
• Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
• Minimum 5 YEARS  of experienced required.

Preferred Qualifications:

• Experience in cardiology or family medicine.
• Bilingual proficiency in English and Spanish.
• Familiarity with electronic Criterions charting systems and quality patient care protocols.

Responsibilities:

• Conduct thorough medical histories and physical examinations to assess patient health.
• Order and interpret diagnostic tests, including laboratory procedures and stress tests, etc.
• Develop and implement individualized treatment plans, including medication management and patient education.
• Perform routine follow-ups and medication reconciliation to ensure adherence and address any concerns.
• Document patient interactions and care plans accurately in electronic health records.

Skills:

The required skills in medical history taking and clinical assessments are essential for accurately diagnosing and treating patients. Medication management and adherence are critical in ensuring that patients follow their prescribed treatment plans, which directly impacts their health outcomes. Knowledge of medical terminology and procedures allows for effective communication with both patients and healthcare team members. Proficiency in electronic charting enhances the efficiency of patient documentation and care coordination. Additionally, skills in patient education empower individuals to take an active role in their health management, particularly in the context of cardiovascular disease.
