Home Health Registered Nurse

Position Overview:
The Home Health Registered Nurse provides skilled nursing services to patients in their homes across multiple counties. This role focuses on patient assessment, care coordination, communication with providers, and accurate documentation while meeting established productivity expectations.

Schedule & Work Details:

• Weekly Hour Guarantee: 30-40 hours
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Productivity Expectation: 25-30 points
• On Call Requirement: Rotating weekend on-call
• Coverage Area: Lake, LaPorte, Starke, and Marshall Counties
• Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
• Pay Rate: $51-$53 per hour

Key Responsibilities:

• Assess patients in the home setting and monitor changes in condition
• Communicate patient status, care needs, and updates with physicians and interdisciplinary team members
• Take and process physician orders accurately and timely
• Participate in weekly meetings and case coordination discussions
• Complete required documentation and comply with all home health standards and policies

Required Qualifications:

• Associate degree or higher
• Current BLS certification

Preferred Qualifications:

• Experience with Homecare Homebase (HCHB)
• OASIS training or experience
