If you're passionate about delivering exceptional care, enjoy working independently while being part of a supportive team, and want to make a lasting impact in your community, we'd love to meet you.As an RN Case Manager, you'll have the opportunity to care for patients throughout their healthcare journey—developing personalized care plans, coordinating with an interdisciplinary team, and helping patients safely recover where they feel most comfortable: at home.If you're looking for a place where you'll be appreciated, supported, and inspired to make a meaningful difference every day, we'd love to welcome you to the Interim family.Our Home Health Registered Nurses enjoy some notable benefits:$33-40/hour1:1 nurse-to-patient ratios where you impact outcomesFlexible assignments, autonomy and work-life balanceOnline training, growth and ability to earn CEUsPTO, Holiday Pay, Medical & 401(k)Part time availableWhat You'll DoAs an RN Case Manager, you'll be trusted to provide skilled nursing care while coordinating all aspects of your patients' care.Your responsibilities will include:Conduct comprehensive nursing assessments and develop individualized plans of care.Manage a caseload of home health patients from admission through discharge.Coordinate care with physicians, therapists, social workers, home health aides, and other members of the healthcare team.Provide skilled nursing visits, education, and disease management.Complete accurate and timely OASIS assessments and clinical documentation.Educate patients and families to promote independence and confidence in managing their health.Monitor patient progress and adjust care plans as needed.Advocate for patients while ensuring the highest standards of quality, safety, and compassion.Participate in case conferences and collaborate with the interdisciplinary team to achieve the best possible outcomes.What We're Looking ForWe're searching for someone who is:A licensed Registered Nurse (RN) in South Dakota.Passionate about delivering patient-centered care.Organized, self-motivated, and able to work independently.An excellent communicator with strong critical thinking skills.Comfortable using electronic medical records and completing clinical documentation.Dedicated to teamwork, collaboration, and continuous learning.Home Health experience is preferred but not required.
