Outreach Case ManagerClassification Reports To Date RevisedExempt SOP Director 9/2026Role: The Outreach Case Manager will provide intensive case management, including specialized trauma-informed and evidence-based interventions with youth up to the ages of 26 years old. The individual in this position will create a safe and welcoming atmosphere for youth being referred to and enrolled in program services, with a focus on creating a positive relationship to engage and retain youth in program services. These individuals will be responsible for implementing evidence-based interventions with program fidelity and comfortable providing services in non-traditional settings.Description of Duties and Responsibilities:Create a caring, safe, and non-judgmental atmosphere, keeping the youth at the center, throughout their experience with the Street Outreach Program.As a member of the outreach team, conduct and document daily street outreach in the community, as outlined by the program outreach schedule.Assist youth with completing enrollment, discharge, and follow-up forms and assessments as well as any additionally required agency and grant paperwork.Maintain flexible communication with the youth served (face-to-face in community, phone, email, text, and/or warm hand-offs at an office location with other referral agencies) to engage the youth, build a positive relationship, and meet their needs.Maintain weekly communication and meetings with clients at the location of their choice, or as determined appropriate by the team.Develop and maintain positive relationships with community organizations to promote SOP services, strengthen referral partnerships, and connect youth with needed and available community resources including housing agencies.Provide evidence-based interventions as appropriate and with fidelity.Maintain a caseload and provide intensive case management services to assist in navigating other service systems to secure resources such as housing, transportation, food, and other practical items to aid youth in accomplishing their goals.Provide transportation for clients to work, school, appointments, and other locations as appropriate.Follow-up with clients following discharge, providing resources and support as appropriate.Document client data accurately and completely, in accordance with Crittenton Center and ACF/SAMHSA project documentation guidelines.Work with program director and staff to achieve and maintain client caseloads.Participate in SOP meetings, retreats, training, outreach, community events, and other project activities as assigned.Maintain client and employee confidentiality, abiding by all state and federal confidentiality expectations including those established by HIPAA.Complete all client documentation in the timelines required.Contribute to the overall success of the program by performing other duties and responsibilities as assigned.Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.CompetenciesTrauma Informed Care & Evidence Based InterventionsCase Management & AdvocacyRelationship BuildingCommunity Collaboration and NetworkingCommunication & DocumentationFlexibility and AdaptabilityTime Management & Organizational SkillsEthical Standards & ConfidentialityWork EnvironmentThis job operates in an office setting, with much of the work taking place in the community. The role requires the ability to sit and concentrate for prolonged periods of time.
