Responsibilities As a Home Health Physical Therapist, you will: Perform physical therapy evaluations and treatment plans based on physician orders Recommend and assist with adaptive equipment as needed Establish measurable patient goals and individualized treatment plans Help patients improve mobility, strength, balance, and independence Maintain ongoing communication with physicians, caregivers, patients, and interdisciplinary teams Complete accurate and timely documentation to support quality care and billing requirements Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with state practice regulations Educate patients and family members on treatment plans, safety, and home exercise programs Qualifications Bachelor’s Degree or higher in Physical Therapy from an accredited program Active Physical Therapist (PT) license in the state of practice Current CPR certification required Minimum of two (2) years of Physical Therapy experience preferred Home Health, community-based, or hospice experience preferred New graduates may be considered in select markets Strong communication, organizational, and patient care skills Valid driver’s license and reliable transportation required Employment Type: Full Time Years Experience: 1 - 3 years Salary: $70 - $75 Hourly Bonus/Commission: Yes
