Position: Home Health Occupational Therapist

Location: Seaside, OR and Surrounding Areas

Job Description

We are seeking a dedicated Home Health Occupational Therapist to join our team in Seaside, OR, and the surrounding areas. The ideal candidate will have a valid Oregon occupational therapy license and a minimum of one year of skilled home health experience.

Shift Details

• Shift Type: Day 5x8-Hour (08:00 - 16:30)
• On-Call Requirement: No
• Weekend Requirement: No

Responsibilities

• Provide one-on-one patient care in a home health setting.
• Conduct assessments and develop individualized treatment plans.
• May visit up to 6 patients per day, with a standard productivity goal of 30 points.
• Utilize Oasis for patient documentation and collaborate with a multidisciplinary team.

Qualifications

• Must hold a current Oregon occupational therapy license.
• 1 year of skilled home health experience is required.
• BLS certification is required.
• Experience with Oasis documentation is preferred; please indicate if lacking this experience.

Special Requirements

• Must have own transportation; the role may involve driving up to 60 miles daily.
• Facility requires primary series COVID vaccination.

Work Environment

• Type of Facility: Home Health
• Attire: Business casual; scrubs are allowed but not required.
• Charting System: EPIC Remote Client for Home Health (on laptop) and Rover (phone app).
• Parking Cost: Free
• Mileage Reimbursement: IRS standard rates apply.
