Medical Social Worker For Home Healthcare

Part-Time/Per Diem Medical Social WorkerPhysicians Choice Home Health is seeking a part-time/per diem Medical Social Worker to join our home healthcare team. In this role, you'll provide in-home social work visits to patients and their families, helping them navigate the emotional, social, and environmental challenges related to their care. You'll work closely with nurses, therapists, and a supportive clinical team to coordinate services and connect patients with the resources they need.Physicians Choice is part of Obran Cooperative, a worker-owned healthcare organization where clinicians have a real voice, shared ownership, and a long-term stake in the success of the company.What You'll DoProvide in-home social work visits to assess patients' psychosocial needsEvaluate patients' living situations, support systems, and access to resourcesDevelop and implement care plans to support patient well-being and independenceConnect patients and caregivers with community resources and support servicesEducate patients and families on available services and care optionsCollaborate with nurses, therapists, and the care team to coordinate patient careCommunicate relevant social and emotional factors impacting patient healthDocument visits, assessments, and care plans in a timely mannerParticipate in care planning, case conferences, and discharge planningOther duties as assigned by clinical leadership
