Your success in this role is defined by your ability to: + **Lead compassionate patient care** by delivering clinical interventions that align with our high standards of safety and excellence, ensuring every patient feels seen and supported. + **Drive personalized care plans** by continuously monitoring and adjusting patient strategies to ensure recovery stays on track and clinical outcomes are met. + **Champion clear communication** by documenting and reporting changes in patient status in real-time, acting as the vital link between the patient and the wider medical team. + **Coordinate the multidisciplinary journey** as the primary point of contact, harmonizing input from various specialists to provide a seamless, holistic healing experience. + **Facilitate safe transitions** by proactively managing discharge planning from day one, ensuring patients and their families are empowered and prepared for the next stage of their recovery. **What minimum requirements you'll need** Licensure / Certification / Registration: + BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. + Registered Nurse credentialed from the Oklahoma Board of Nursing required.
