Job Summary The Case Coordinator is responsible for incorporating several professional functions into the management of the total episode of care for patients on their assigned unit(s). She/he uses independent judgment in connection with her/his managerial responsibilities. Department Care Coordination Location Englewood, NJ Status Full Time Shift 8 hours Hours 9a-5p Job Essentials Visiting Nurse or Case Management experience preferred or 5 years' acute care Nursing experience required.
