Center for Key Populations Medical Assistant/LPN - 8 hours

Job Description Summary Part time and grant funded opportunity Tuesdays 8am-5pm. During Spring and Summer includes one Saturday a month; during Fall and Winter includes one Sunday a month. Organization The Center for Key Populations (CKP) is a program of Community Health Center (CHC), Connecticut's largest federally qualified health center. Role Overview As an LPN at Community Health Center, you will provide clinical and technical support for our primary care providers, ensuring efficient and accurate workflow through the use of electronic tools. Responsibilities include monitoring equipment and performing clinical testing in accordance with OSHA and CLIA standards, acting as a liaison with patients, families, specialists, and all members of the health care team, and supporting medication administration, immunizations, and other provider clinic tasks within the scope of your LPN licensure. As a Medical Assistant, you will provide similar clinical and technical support, implementing all planned care items, monitoring equipment and testing, liaising with patients and the health care team, and ensuring continuity and coordination of care in a patient‑centered medical home. Responsibilities Implement planned care items using electronic tools to ensure efficient and accurate workflow. Monitor equipment and perform clinical testing according to OSHA and CLIA standards. Act as a liaison with patients, families, specialists, and all members of the health care team. Assist primary care nurse and provider to ensure continuity and coordination of care. Administer routine medication, immunizations, and other provider clinic support within the scope of LPN licensure. Apply evidence-based guidelines from major regulatory and specialty groups. Prepare patients for visits, including guiding them on use of video telehealth technology. Manage virtual telehealth sessions for multiple providers using Zoom technology. Support clinical teams in paperwork collection, records requests, scheduling of follow-up visits, and other administrative tasks. Collaborate with providers, clinical, and clerical staff to facilitate appropriate telehealth patient flow. Accurately document chief complaints and results from evidence-based screening tests. Record patient vital signs and perform CLIA waived tests, EKG, hearing, and vision tests independently and accurately. Prioritize work responsibilities daily to reduce waiting time. Schedule tests and referrals for specialty care per provider orders in a timely manner. Maintain a clean, orderly, safe working environment and oversee supplies and medical logs. Set up and stock exam rooms and clean rooms at the end of each day worked. Adhere to infection control standards, Universal Precautions, and agency safety policies. Maintain yearly CPR certification and other required qualifications. Exhibit excellent customer service skills to external and internal customers. Comply with agency mandated in‑service training workshops. Qualifications and Skills LPN Program Completion Certificate (current CT LPN License). Experience with EKG use, appointment scheduling, word processing, and computer skills. Familiarity with medical testing procedures and OSHA/CLIA compliance. Sound organizational skills and accuracy at all levels of work. Excellent communication and customer service skills. Team-focused with demonstrated leadership qualities. Preferred: Bilingual; expert knowledge of the electronic health record. Required Licenses/Certifications: CPR – American Red Cross CPR for the Professional Rescuer and AED certification or American Heart Association BLS/AED for Healthcare Providers. Medical Assistant—Qualifications High school diploma or equivalency. Completion of a Medical Assistant course from an accredited/recognized school. Medical Assistant board certification/registration (required). Knowledge of EKG use, appointment scheduling, word processing, and computer skills. Prior demonstrated excellent communication and customer service skills. Team‑focused with demonstrated leadership qualities. Required CPR certification (American Red Cross CPR for the Professional Rescuer and AED; alternative: American Heart Association BLS/AED for Healthcare Providers). Preferred: Bilingual; expert knowledge of the electronic health record. Physical Requirements and Work Environment Requires manual dexterity, moderate physical activity including standing, walking, and lifting. Potential exposure to blood‑borne pathogens and/or bio‑hazardous material. Confidentiality Full access to patients' medical records and accounts.
