Role Description This full-time, on-site Physical Therapist role is based in Plaistow, NH and focuses on delivering direct patient care in an outpatient clinical setting. Day-to-day responsibilities include performing evaluations, developing individualized treatment plans, and implementing therapeutic interventions to address pain, mobility limitations, and functional deficits. The Physical Therapist will provide hands‑on therapy, patient education, and home exercise programs, while monitoring progress and adjusting plans as needed. The role also involves collaborating with referring providers and other healthcare professionals, documenting treatment in accordance with professional and regulatory standards, and supporting a safe, welcoming environment for all patients. Qualifications Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement evidence-based treatment plans. Experience in Patient Care and Home Care, including patient education, home exercise instruction, and functional training. Knowledge of Medicine related to musculoskeletal, neurological, and orthopedic conditions relevant to physical therapy practice. Active state license (or eligibility for licensure) as a Physical Therapist in New Hampshire. Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program. Effective communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team. Strong documentation, time management, and organizational abilities.
