A Behavior Health Nurse - RN provides nursing care to chronically or acutely sick patients; provides medication monitoring, adjusts medications within limits prescribed by Medical Director or designate, and performs in liaison role between client and Medical Director or designate, lab, pharmacist, family or residential facility staff, and other community caregivers; provides information, education and support for clients; promotes wellness, recovery and skill building to sustaining better health; prevention training in the areas of wellness, recovery, managing symptoms, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery.

Essential Duties & Responsibilities

• Provides life skills and wellness didactic services utilizing evidence-based methodologies to provide excellent clinical services. Prepare for and conduct individual, group, and family therapy for assigned caseload.
• Assist service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered. Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned. Update treatment plans as appropriate. Submit treatment plans at all stages for medical review, if applicable.
• Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conduct assessment and write assessment summaries, as assigned.
• Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. And maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
• Assist service recipient in making discharge plans and provide follow up medical services as assigned.
• Administers and gives medications to patients, monitors the dosage, side effects, use and documents patients' reactions. Provides medication training to the client or client's family or guardian to administer the client's medication, to monitor proper levels of dosage and watch for side effects. Obtains laboratory specimens and help doctors with patient examination and continuity of care arrangements with other providers.
• Monitor urinalysis, breathalyzer, and other testing as assigned.

Knowledge, Skills & Abilities

• Effectively communicate via written, verbal, in person and virtual methods.
• Skilled in customer service techniques.
• Skilled in problem-solving.

Qualifications

• Education Level: Minimum of Associate's Degree in Nursing
• Years of Experience: Must meet Qualified Mental Health Professional status in relevant programs.
• Certification/Licensure: Active unencumbered Illinois Registered Nurse required.

Pay Range: $29.63--$41.54
