with a minimum commitment of two shifts per month Outpatient-only setting Cover provider PTO and sick leave at both SSM Health DePaul Hospital and SSM Health St. Joseph West Hospital Strong onboarding, mentorship, and provider support Highly collaborative team culture Competitive hourly rate and malpractice coverage What You'll Do: Conduct pre-surgical evaluations and determine optimization needs using established clinical pathways Collaborate closely with primary care providers and specialists to coordinate patient care Evaluate patients following hospital discharge and provide timely assessment, treatment, or specialist referrals as needed Focus on clinical evaluation and care coordination with limited diagnostic workups and prescribing responsibilities Qualifications: Master's degree as a Nurse Practitioner or Physician Assistant Minimum of

2 years of current experience

in Internal Medicine or Adult Primary Care Missouri NP licensure or Physician Assistant certification/licensure Strong assessment, clinical judgment, and patient management skills If you're an experienced APP who enjoys care coordination, patient education, and improving clinical outcomes in a supportive outpatient environment, we'd love to connect.
