This is more than a traditional outpatient role. You'll work directly within a housing community, building meaningful connections with residents, providing clinical support, crisis intervention, assessments, treatment planning, and helping people navigate their recovery journey at their own pace. Location: Bud Clark Commons & Old Town Recovery Center Schedule: Monday-Friday 8:00am-4:30pm (Saturday & Sunday off) Salary Ranges from: $69,000–$124,000. Final compensation will be determined based on relevant experience, education, credentials/licensure, and internal equity.

MINIMIUM QUALIFICATIONS: QMHP-C

LMFT-A or LMFT LPC-A or LPC CSWA or LCSW CADC-1 at minimum HIGHLY preferred. Experience with severe and persistent mental illness (SPMI) preferred. Has the competencies necessary to: Identify precipitating events Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts Assess family, social and work relationships Conduct a mental status assessment Document a DSM diagnosis Write and implement or supervise implementation of a treatment plan Conduct and document a mental health assessment Provide mental health treatment and rehabilitative services within one's scope of practice Must meet CCC privileging requirements as required by FTCA. CADC-R at minimum preferred. Current CPR certification required prior to start. Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern. Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988. Must pass a pre‑employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit. Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects. Must be able to climb stairs several times a day. Must adhere to agency's non‑discrimination policies. Ability to effectively interact with co‑workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life‑styles and sexual orientations, and treat individuals with respect and dignity. Preferred Certified Directors Custody designee in Multnomah County or ability to obtain certification. ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide engagement services to clients who are pre‑contemplative regarding recovery of mental illness, addictions and health issues utilizing strengths‑based case management and motivational interviewing concepts. Coach clients in setting goals and identifying realistic steps in order to achieve them. Assist clients in developing and accessing natural community supports. Advocate for each individual when there are difficulties obtaining needed services in the community. Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients. Assist with milieu management of the common areas in the upstairs Bud Clark Commons, providing direct milieu‑based services to clients while modeling clinical skills to other staff. Provide individual and group treatment to clients utilizing illness management and recovery concepts. Assist in crises as appropriate, including assessing risk of harm, following emergency protocols/procedures and coordinating with the treatment team with interventions, which support a trauma informed environment for residents and staff. Monitor and assure that the health and safety needs of clients are met. Participate in critical incident reporting and critical incident debriefings as needed. Teach and model positive behaviors and helps clients reestablish sense of self and personal aspirations. Demonstrate responsibility for safety of residents, staff and property; is familiar with fire regulations and evacuation procedures. Communicating important resident information to Home Forward staff. Participate in at least two (2) hours of supervision per month. The two hours must include one hour of face‑to‑face contact for each person supervised, or a proportional level of supervision for part‑time staff providing clinical or other support services to clients. Supervision may take place in the community during clinical interactions. Collaborate with Residential facilities, Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and non‑residential treatment and service planning as needed. Participate in admission and discharge planning for enrolled clients needing hospitalization or subacute level of care. Contact for enrolled clients must be made within one working day of admission. Participate in outreach activities as necessary to offer services and engage participation in a non‑coercive manner. Escort clients to meetings and appointments as necessary. Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent. Conduct and document Mental Health Assessments for enrolled clients, including mental status exam, DSM diagnosis, and clinical formulation. Develop service plan with enrolled clients. Maintain client records, using an electronic medical record, as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments & individual service plans. Attend required meetings and trainings. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by +CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. Conduct educational and therapeutic groups for clients, using approved curriculum. Coordinate services with all members of client's support network. Participate in quality assurance and quality improvement activities as directed. Assist with the development of an integrated treatment program. Perform other duties as assigned. SKILLS AND ABILITIES: Advanced knowledge and skills in counseling and assessment techniques with individuals and groups including assessing for risk of danger to self & others. Knowledge and skills in mental health treatment methods, Evidenced Based Practices and crisis intervention. Advanced knowledge and skills in substance use treatment methods and crisis intervention. Knowledge & understanding of the value of the integration of behavioral health, primary care & substance use services. Knowledge of de‑escalation methods or ability to be trained in de‑escalation methods. Working knowledge of Integrated Dual Diagnosis, Recovery Models of treatment, and Trauma Informed Care. Computer skills necessary to accomplish duties including ability to document in an electronic health record. Ability to work effectively and efficiently in an environment subject to ambiguity and change. Ability to work effectively with peers, individual clients and groups. Ability to work as a team member, remain flexible and to establish priorities as a group. Ability to apply principles learned to new situations. Ability to maintain accurate records and necessary paperwork. Ability to communicate clearly and concisely, both orally and in writing. Ability to be a creative, innovative, and progressive problem‑solver. Ability to work effectively with a diverse population. Ability to work independently, manage time, prioritize the workload, problem‑solve, and complete tasks in a timely fashion. Ability to multi‑task and remain calm in a busy and stressful environment. Ability to be a detail oriented, flexible, dependable, positive team player. Ability to display a non‑judgmental attitude. Knowledge of community and social service agencies. Sufficient manual dexterity and physical ability to perform assigned tasks. Ability to work courteously and effectively communicate with the general public, clients, medical personnel, corrections personnel, police, and co‑workers.
