Analyze patients' nutritional status to create nutrition care plans, monitor dietary intake and weights, and adjust as needed to support optimal nutritional outcomes. Complete initial, annual, quarterly, and high-risk assessments per hospital policy to ensure timely evaluation of patient needs. Record diet histories and assessments, charts, and progress notes to maintain accurate clinical documentation. Monitor food service operations to ensure nutritional adequacy and safe food handling practices. Ensure regulatory requirements set by Centers for Medicare/Medicaid Services (CMS) and The Joint Commission (TJC) are being met by following standardized processes. Implement evidence-based practice, interventions, and recommendations using Nutrition Care Manual Standardized Terminology into daily practice to support consistent, high-quality care. Work as part of a collaborative healthcare team to ensure optimal nutritional results. Ability to analyze patients' nutritional status, dietary intake, and weight trends. Ability to implement evidence-based practice, interventions, and recommendations using Nutrition Care Manual Standardized Terminology. Ability to monitor food service operations to ensure nutritional adequacy and safe food handling practices. Ability to ensure regulatory requirements set by CMS and TJC are being met by following standardized processes. Ability to create nutrition care plans and adjust them as needed. Ability to complete initial, annual, quarterly, and high-risk assessments per hospital policy. Ability to work as part of a collaborative healthcare team to ensure optimal nutritional results. Ability to evaluate and monitor patient nutritional needs and adjust care plans to achieve desired outcomes. Ability to ensure safe food handling practices and regulatory compliance to support high-quality patient care. Ability to record diet histories and assessments, charts, and progress notes accurately. Ability to manage multiple assessment timelines (initial, annual, quarterly, high-risk) in accordance with hospital policy. Individuals should have the ability to perform essential functions with or without reasonable accommodation.
