In this role, you'll be the friendly voice on the other end of the phone—connecting members to the care, services, and support they need to live healthier lives. What You'll Do Make outbound and receive inbound calls with members and healthcare partners. Build positive relationships through compassionate, professional communication. Assist Tailored Care Managers by completing outreach, scheduling appointments, coordinating services, and documenting member interactions. Help connect members with healthcare providers, community resources, and essential support services. Maintain accurate documentation while meeting quality and productivity goals.
