Case Manager (part-time) - New Covenant Center

Provide compassionate case management for individuals and families in crisis, focusing on crisis intervention, advocacy, and connecting clients to internal and external resources. Accountabilities Conduct intake interviews and needs assessments to determine program eligibility. Develop service plans to support crisis resolution and self-sufficiency. Connect clients with community resources and internal programs. Track client progress and maintain accurate case notes and documentation. Supervise Café floor during meals and cross-train in other Operations areas. Build collaborative relationships with agencies, service providers, and community partners. Education & Experience High school diploma, two-year college degree, or equivalent relevant experience. 2+ years experience in case management, assessment, counseling, or crisis intervention preferred. Bilingual English/Spanish strongly preferred. Proficient in Office 365; organized, detail-oriented, and able to manage multiple tasks.
