RN Care Case Coordinator Home Care

Position Overview Cheer Home Care is looking for a RN Care Case Coordinator who loves spending time with older adults and provides high quality care in home. The RN Care Case Coordinator collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, caregivers, and nurses. The role involves working closely with our office team and requires excellent communication and problem‑solving skills. Primary care manager, coordinating care among caregivers, RNs, therapists, and outside representatives. Option to work one weekend day and have one weekday off.
