Physical Therapist

Role: Physical Therapist focused on delivering in-home and senior living care to older adults, utilizing a flexible, patient-centered approach within a Geriatric House Calls model. Responsibilities: Conduct evaluations, develop personalized treatment plans, provide evidence-based therapy, support patient mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate. Qualifications & Skills: Valid state license or eligibility, degree from an accredited program, proficiency in EMR and basic computer skills, current CPR certification, and ability to meet physical demands such as lifting, repositioning, and driving.
