• Guaranteed Hours: 40

Requirements:

• Current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from the American Speech-Language-Hearing Association (ASHA) or CCC eligible.
• Current state license as applicable.
• Proof of current CPR certification.
• Current driver's license.
• Reliable transportation and insurance.

Roles & Responsibilities:

• Conduct evaluations and assess the home environment to identify equipment needs related to speech/language function.
• Identify functional speech defects and establish a Plan of Care (POC) to enhance the patient's functionality.
• Administer speech therapy programs utilizing specialized therapeutic techniques and/or equipment.
• Recommend communication devices/aids as indicated.
• Collaborate with referring physicians and clinical staff to ensure coordinated care contributes to the total Plan of Care (POC).
• Document patient care progress and the continuing need for treatment.
• Educate patients and families/caregivers on tasks and resources that support continued functional independence.
• Identify needs for additional services and refer as appropriate.
• May supervise home health aides in procedures related to the speech-language therapy Plan of Care (POC).
• Participate in case conferences and meetings to ensure coordinated care.
