Overview Are you a dedicated Licensed Master Social Worker (LMSW) who believes compassionate care can change lives? As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times. Connect Hearts and Homes: Evaluate medical needs, home situations, financial resources, and community support. Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator. Be part of a United Team Effort: Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses. Provide Counseling with Compassion: Provide empathetic counseling to patients and families. Qualifications Education Required: Master of Social Work from an accredited school of social work License Requirements Required: Licensed as a Social Worker from the State Board of Social Work Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle Experience Preferred: Minimum one (1) year experience as a social worker in a hospice setting Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred Additional State-Specific Requirements: AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX: Current unencumberedlicense as a Social Worker from the State Board of Social Work $00.00 - $00.00 per hour (Average Pay Range).
