In this role, you will provide clinical assessments, individual and group therapy, treatment planning, case management, and discharge coordination for patients receiving inpatient behavioral health services.If you're passionate about helping individuals through some of life's most challenging moments while working alongside a supportive clinical team, we invite you to apply.What You'll DoAs a Patient Care Manager (PCM), you will:Complete comprehensive psychosocial assessments.Develop, implement, and update individualized treatment plans.Facilitate individual, family, and group therapy sessions.Coordinate discharge planning and connect patients with community resources.Collaborate with psychiatrists, nurses, behavioral health technicians, and other members of the interdisciplinary treatment team.Document assessments, treatment plans, and patient progress in the electronic medical record.Participate in utilization review and ensure timely documentation.Advocate for patients while maintaining the highest standards of confidentiality and ethical practice.Assist patients and families in achieving treatment goals and successful transitions of care.QualificationsEducationMaster 's degree in social work, Counseling, Psychology, or another behavioral health-related field.LicensureWe welcome applicants at various stages of licensure:LMSWLCSWLACLPCLAMFTLMFTLASACClinical supervision is available for associate-level clinicians.RequirementsCurrent CPR certification (or obtained within 30 days of hire)Current TB clearanceArizona Fingerprint Clearance CardMust be at least 21 years of ageWhy Aurora Behavioral Health?At Aurora, you'll become part of a mission-driven organization dedicated to restoring hope and promoting healing.
