What is the license requirement to bid?: Licensed in CA only

Preferred experience in long term care and geriatric medicine/experience in geriatric adult medicine. NP will be working independently so please ensure they have the number of years of experience to do so. They will not be 100% on their own but their day to day work in the clinic will be independent. ONLY NP's will be considered.

They will work in the clinic for 3 days then will spend the other days doing documentation, home visits, or skilled nursing facility visits. 90% of the work is clinic based, 10% is home visits and nursing home visits. This is a FULL TIME need so we do prefer to find one provider to work the entire job but we will consider two if need be. The need could potential extend depending on recruitment.

Team based model of care (please see support staff details in the AR, there will be a lot of support in the clinic).

No privileges are required, but the provider will need to get set up with EPIC access and onboarding before starting.

CALL IS NOT REQUIRED. There is an option for the provider to help with call if they want to. This can be discussed further with the site.

- Call is nights and weekend (Nights is 5pm-8am, weekends are 8am Saturday-8am Monday).

- It is beeper call (after hours nurse usually answers first or the operations director gets a call and then calls the provider
