Field Registered Nurse

Benefits: Training & development Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Registered Nurse for a PRN field position serving patients throughout South Dallas, including DeSoto, Duncanville, Oak Cliff, Lancaster, and surrounding communities. Why This Position Matters Our Registered Nurses provide skilled, patient-centered care in the comfort of each patient's home. This position helps patients safely manage their health conditions while supporting their independence and quality of life. Responsibilities Perform skilled nursing visits in patients' homes Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits as assigned Conduct comprehensive patient assessments Develop and update individualized plans of care Administer medications and treatments according to physician orders Provide wound care, IV therapy, disease management, and other skilled nursing services as ordered Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans Monitor patients for changes in condition and promptly communicate concerns Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team Complete accurate and timely OASIS and clinical documentation Follow agency policies and applicable Medicare, state, and federal requirements Travel to patients throughout DeSoto, Duncanville, Oak Cliff, Lancaster, and surrounding areas Qualifications Active and unrestricted Texas Registered Nurse license Current CPR or BLS certification At least one year of nursing experience preferred Home health, hospice, or palliative care experience preferred OASIS and WellSky/Kinnser experience is a plus Strong assessment, clinical decision-making, and time-management skills Proficiency with electronic medical record systems Valid Texas driver's license Reliable transportation and current automobile insurance Ability to work independently and collaborate with the clinical team Why Join Amcare Pro?
