As a successful and growing agency, Holyoke VNA & Hospice Life Care is looking for an experienced person. Compassionate and detail-orientated Registered Nurse who wants to service our community in a part-time and on-call capacity. The Weekend Evening /On Call Registered Nurseshift will include Saturday & Sundays, from 4:00pm through 12:00am, with on-call responsibilities from 12:00am – 8:00am. Responsibilities may include the following: Admissions, symptom management visits and/or pronouncement visits. Instruct patients and caregivers in symptom management and end of life care. Communicates with MD to facilitate recommendations for symptoms and pain management. Evaluate outcomes of nursing intervention and initiate changes when appropriate Collaborate, communicate and cooperate as appropriate with other health care providers to ensure quality and continuity of care. Complete, maintain and submit accurate and timely documentation and relevant clinical notes regarding the client’s condition and care and treatment provided. Requirements: Associates Degree (Nursing) required, BSN preferred Nursing experience one (1) to three (3) years is required, preferably in a home care environment.
