Overview Of The Role This position provides PRN care management support, focusing on patient advocacy, safety, and responsible health practices. Identify and address barriers to patients' care plans through interdisciplinary collaboration. Manage care for patients with complex medical conditions, ensuring appropriate intervention levels without duplication. Facilitate safe and timely transitions between care settings, including home, hospital, or post-acute facilities. Work in various environments, utilizing telehealth and virtual tools effectively. Collaborate with inpatient care teams for smooth patient handoffs. Engage patients and families to align care with their values, goals, and functional independence. Utilize community resources and address gaps in care. Maintain knowledge of legal, ethical, and confidentiality standards related to case management. Document activities timely and adhere to departmental policies and standards. Document activities timely and adhere to departmental policies and standards. Qualifications & Skills Registered nurse with a current license; Bachelor\'s degree in Nursing or higher preferred. Case management certification (CCM or RN-BC) preferred or attainable within 36 months. Strong communication, interpersonal, and collaboration skills. Proficiency with Microsoft Office and understanding of managed care principles. Ability to assess, prioritize, and adapt in fast-paced, diverse environments.
