Growing together.The Optum SCC program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Optum SCC SL RN will be managing members in a community setting to include personal homes, group homes, and assisted living facility setting. As one of the largest employers of APCs, Optum offers unparalleled career development opportunities.Primary Responsibilities:Ensure accurate and complete documentationCommunicate and collaborate with the interdisciplinary care teamSupport the telehealth visits in the field, the RN sets up members for telehealth with the APCAssist the provider / team with various care coordination activitiesAssist the provider / team with benefit determination associated with Medicare part A, part B, and part D benefitsResponsible for collaborating with the primary advanced practice clinician (APC) and facility to identify and respond to patient change in conditionAssist the provider / team with therapy coordinationParticipate in facility-based meetingsSchedule and participate in family conferences, team meetings, and team case presentationsCoordination of training, tracking, and compliance with quality measuresAssist in creating and conduct in-services for selected audiencesAssist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.May write verbal orders from Provider in the chart in compliance with the state RN practice laws, and in compliance with individual nursing facility practicesAssist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventionsAssist APC in obtaining compliant related documentation for transfer to stay in a SNFYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Active and unrestricted RN license in ORAssociate degree or higher in Nursing (RN)Certified in Basic Life Support3+ year of clinical experience in practiceAbility to complete physical requirements, including moving a 30-pound bag and ability to perform physical assessmentAccess to reliable transportation and willingness to travel in between member locations dailyProficient computer skills, including the ability to document medical information with written and electronic medical recordsProficient in Microsoft Office products including Word, Excel and TeamsAbility to develop and maintain positive customer relationshipsHighly adaptable to the changing needs of our members dailyEffective communication skills and time managementPreferred Qualifications:1+ year experience working in long-term care, home health, hospice and/or case managementExperience in meeting the medical needs of patients with complex behavioral, social and/or functional needsUnderstanding of older adults and chronic illnessUnderstanding of Advanced Illness and end of life discussionsAbility to work with diverse care teams in a variety of settingsProfessionalism:Personal and Professional AccountabilityCreate an environment that facilitates the team to initiate actions that produce positive resultsAbility to hold self and others accountable for actions and resultsAnswers for one's own behavior and actionsCreate an environment in which professional and personal growth are an expectationIntegrate high ethical standards and UHG core values into everyday work activitiesAbility to provide strong customer service and maintain respectable customer relationshipsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 - $107,400 annually based on full-time employment.
