Physical Therapist

Schedule: 8 hours per shift

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 13

Requirements:

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license. Must have reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document, prepare, and review plans of care, progress notes, and other clinical record documentation.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
• Assist and instruct patients and families in the proper use of therapy equipment and transfer techniques.
• Provide patient and family education regarding therapeutic procedures based on patient characteristics.
• Oversight of other team members in patient care activities as applicable.
• Apply infection control and universal precautions in patient care activities.
• Accept clinical assignments consistent with education and competence.
• Perform other related duties as assigned.
