Oak Medical is seeking a Nurse Practitioner or Physician Assistant to provide onsite care for adult and geriatric patients across our post-acute and long-term care partner communities. You'll work alongside experienced physicians and interdisciplinary care teams while enjoying a predictable weekday schedule and strong clinical support.

Your Impact

As a valued member of our clinical team, you will:

• Provide primary care services for adult and geriatric patients
• Diagnose and manage acute and chronic medical conditions
• Develop and implement individualized treatment plans
• Prescribe medications and perform appropriate clinical procedures
• Coordinate care with facility teams and specialty providers
• Communicate effectively with patients, families, and caregivers
• Maintain accurate and timely documentation
• Help improve quality of life and health outcomes for the patients you serve

Qualifications

Required:

• Licensed Nurse Practitioner or Physician Assistant
• Active DEA registration
• Current BLS certification
• Strong clinical assessment and diagnostic skills

Preferred:

• Experience in geriatrics, skilled nursing, long-term care, hospital medicine, or post-acute care
• Leadership experience
• Strong organizational and time-management skills
