Case Management Registered Nurse

Location: Spring Hill, FL

Assignment Duration: This is a 13-week contract position.

Schedule: 08:30 AM - 05:00 PM with alternating weekends required.

Job Description: We are seeking a dedicated and experienced Case Management Registered Nurse to join our team. In this role, you will collaborate with a multidisciplinary team to manage patient care effectively, ensuring the highest quality and efficiency throughout their journey.

Responsibilities:

Coordinate patient care and collaborate with healthcare professionals.
Assess and manage care plans for patients.
Facilitate communication among patients, families, and healthcare teams.
Ensure utilization of resources and compliance with standards.

Required Experience / Certifications / Licensure:

Must hold a valid Registered Nurse license.
Experience in case management is required.
Familiarity with Meditech and Midas preferred.
