Job Duties/Responsibilities:

• Conduct onsite and telehealth services in our 24 hour, 7-day a week residential facility providing health services in accordance with regulatory (OASAS, DOHMH) and agency requirements
• Health Monitoring including pre-admission and ongoing withdrawal symptom and health screenings, communicating to MD all client medical concerns and health referral needs
• Review all intake health and medical documents, CIWA and COWS assessment tools and consult with Program Director and/or MD for intake result and admission decision, communicating client information with Intake staff
• Health Coordination which ensures the provision of education, risk reduction, counseling and referral services to all residents regarding HIV and AIDS (including pre and post-exposure prophylaxis), tuberculosis, viral hepatitis, sexually transmitted diseases and other communicable diseases
• Document all client’s services in the electronic health records in a timely manner
• Supervise daily tasks of and review all LPN client services documentation to ensure compliance and clinical appropriateness in electronic health records in compliance with Medicaid billing, OASAS/DOHMH and other regulatory requirements
• Conduct monitoring of health indicators identified through a physical examination and/medical or other health screening which will involve individual sessions to determine client medical baseline and medical needs and the creation of health treatment plans in coordination with medical and clinical team
• Supervise all aspects of client medication prescriptions and medication management which includes managing medication logs for medications received from pharmacy, ensure proper medication storage and safety, conduct injections, update and maintain client injection episode in medication log book and ensure all staff are trained in appropriate medication monitoring and storage
• Manage all medical testing services, including ensuring client appointments, testing equipment and materials, pre and post testing lags and reporting to regulatory and/or agency sources as required
• Conduct smoking cessation sessions and monitor client use of NRT supports
• Conduct small group health sessions and focus on educating clients on medication and treatment compliance strategies and educating clients on disease/health disorders
• Conducting intermittent health screenings to measure changes in health status/indicators
• Conduct nutrition, diet and daily activity education and Interventions, alcohol and drug education and other health monitoring activities tracking changes in health indicators and record in client electronic health records

QUALIFICATIONS:

• Valid RN Licensed by NYS Department of Education
• Licensed Registered Nurse, at least three-year post licensure providing nursing care in hospital and/or clinic settings
• Working knowledge and experience working with substance using and co-occurring disorders clients
• Experience conducting health screenings in mental health, health and substance abuse areas
• Experience conducting group health education sessions
• Familiarity with trauma informed environments
• Experience with clinical documentation, use of electronic health records systems and Medicaid billing procedures
• Physical ability to perform the essential job functions is needed.

In addition, the Licensed Registered Nurse will perform any duties as required to meet the needs of the program and the clients that we serve
