LPN/LVN - Acute Rehab

Licensed Practical Nurse (LPN) Acute Rehabilitation

Seeking a compassionate and dependable Licensed Practical Nurse (LPN) Acute Rehabilitation for a 13-week contract to provide direct nursing care for patients recovering from stroke, neurological disorders, orthopedic injuries, trauma, and other medically complex conditions. The LPN will assist with assessments, medication administration, wound care, rehabilitation support, and patient education while collaborating with the interdisciplinary care team to promote recovery and maximize patient independence.

Requirements:

• Current LPN license required (state-specific as applicable)
• BLS certification required
• Minimum 12 years of Acute Rehab, Med Surg, or LTC experience preferred
• Experience caring for post-operative, neurological, orthopedic, and medically complex patients preferred
• Strong assessment, documentation, and communication skills
• Ability to work effectively in a fast-paced interdisciplinary rehabilitation environment
• Compassionate, dependable, and patient-focused with excellent teamwork skills

Responsibilities include monitoring and documenting patient conditions, administering medications and treatments, performing wound care, assisting with mobility and activities of daily living, monitoring vital signs, supporting rehabilitation goals, reinforcing patient and family education, maintaining accurate electronic medical records, collaborating with RNs, therapists, physicians, and the interdisciplinary care team, responding promptly to changes in patient condition, and providing safe, compassionate, and high-quality nursing care throughout the rehabilitation process.
