Social Worker

JOB SUMMARY The Social Worker Assistant is responsible for the implementation of standards of care for social work services under the direct supervision of a Licensed Medical Social Worker. DUTIES & RESPONSIBILITIES Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse. Carries out social evaluations and plans intervention based on evaluation findings. Maintains clinical records on all patients referred to social work. Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs. Provides information to patients or families/caregivers and community agencies. Serves as liaison between patients or families/caregivers and community agencies. Maintains collaborative relationships with organization personnel to support patient care. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel. Participates in the development of the total plan of care and case conferences as required under the direction and oversight of the Licensed Medical Social Worker. Participates in discharge planning in collaboration with a Licensed Medical Social Worker. Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct. Assists physician and other team members in understanding significant social and emotional factors related to health problems. Participates in the HHA’s quality assessment and performance improvement program and HHA-sponsored in‑service training. Other duties as delegated by the Licensed Medical Social Worker. JOB REQUIREMENTS Must have a bachelor’s degree in social work, psychology, sociology, or another field related to social work from a school of social work accredited by the Council on Social Work Education. Minimum of one year of social work experience in a health care setting. If no bachelor’s degree, must have a minimum of two years of appropriate experience as a social work assistant and achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service. Experience in a home health care setting preferred. Demonstrates good verbal and written communication and organizational skills. Possesses and maintains current CPR certification. Has the ability to drive and/or access transportation for agency‑related travel (e.g., community partners, patients). Must have and maintain a valid driver’s license, automobile insurance coverage, and access to an automobile.
